ABSTRACT

Objective:To investigate the effects of removable periodontal splint combined with minocycline on periodontal indexes and tooth aesthetics in patients with severe periodontal disease.Methods:A total of 102 patients with severe periodontal disease treated in the School and Hospital of Stomatology, China Medical University from November 2018 to April 2020 were included in this study. They were randomly allocated into study and control groups ( n = 51/group). The control group was subject to repair with removable periodontal splint based on routine interventions. The study group was subject to medication with minocycline in addition to the treatments used in the control group. Clinical efficacy, periodontal status (sulcus bleeding index, plaque index, periodontal pocket depth) and gingival crevicular fluid inflammatory factors (transforming growth factor β, monocyte chemoattractant protein-1, interleukin-6, matrix metalloproteinase-8) and bone metabolism indexes [osteocalcin, N-terminal procollagen of type I (PINP), N-terminal propeptide of type I procollagen (PINP), C-terminal telopeptide of type I collagen (CTX) levels], comfort and aesthetics scores, and patient satisfaction were compared between the two groups. Results:Total response rate was significantly higher in the study group than in the control group [94.12% (48/51) vs. 80.39% (41/51), χ2 = 4.32, P < 0.05]. At 1 and 3 months after treatment, sulcus bleeding index (1.32 ± 0.41, 1.11 ± 0.36), plaque index (1.51 ± 0.44, 1.32 ± 0.51), periodontal pocket depth [(3.29 ± 0.70) mm, (2.51 ± 0.63) mm] were significantly lower in the study group than in the control group [1.65 ± 0.39, 1.45 ± 0.38, 1.92 ± 0.42, 1.88 ± 0.49, (5.05 ± 0.79) mm, (3.82 ± 0.86) mm, t = 4.16, 4.63, 4.81, 5.65, 11.90, 8.77, all P < 0.001]. At 1 and 3 months after treatment, the level of transforming growth factor β in the gingival crevicular fluid was significantly higher, and the level of matrix metalloproteinase-8 in the gingival crevicular fluid was significantly lower, in the study group compared with the control group (both P < 0.001). At 1 and 3 months after treatment, the level of osteocalcin in the gingival crevicular fluid was significantly higher, and the level of C-terminal telopeptide of type I collagen in the gingival crevicular fluid was significantly lower, in the study group compared with the control group ( t = -9.97, -10.71, -5.77, -7.40, 7.24, 16.11, all P < 0.001). At 1 and 3 months after treatment, the scores of comfort and aesthetics in the study group were significantly higher than those in the control group ( t = 7.49, 8.26, 7.84, 9.10, all P < 0.001). Patient satisfaction in the study group was significantly higher than that in the control group (94.12% vs. 80.39%, χ2 = 4.32, P < 0.05). Conclusion:Repair with a removable periodontal splint combined with minocycline can increase the therapeutic effects through reducing periodontal inflammation and regulating bone metabolism, thereby improving the periodontal condition, and improving tooth comfort and aesthetics and patient satisfaction in patients with severe periodontal disease.

ABSTRACT

Objective:To study the facial aesthetic effect of different periodontal splints combined with orthodontic treatment for patients with sector displacement of periodontitis teeth.Methods:A total of 186 patients with sector displacement of periodontitis teeth admitted to Hunan Second People′s Hospital from April 2018 to April 2020 were prospectively selected as the research objects. 186 patients were divided into observation group and control group by random number table method, with 93 cases in each group. Both groups were treated with orthodontic correction. The observation group was treated with orthodontic combined with super fiberglass periodontal splint, while the control group was treated with orthodontic combined with conventional periodontal splint. The treatment effect, serum inflammatory factors [tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), interleukin-6 (IL-6) ], periodontal pocket depth, bleeding index and plaque index before and after treatment were compared between the two groups, and the satisfaction of the two groups was compared.Results:At half year and 1 year after treatment, the effective rate of the observation group was 91.40%(85/93) and 100%(93/93), which were significantly better than those of the control group [84.95%(79/93), 95.70%(89/93), all P<0.05]. The levels of serum TNF-α, CRP and IL-6 in the two groups at half year and 1 year after treatment were significantly lower than those before treatment (all P<0.05); the levels of serum TNF-α, CRP and IL-6 in the observation group was significantly lower than the control group, with statistically significant difference (all P<0.05). The periodontal pocket depth, bleeding index and plaque index of the two groups after treatment were also significantly lower than those before treatment (all P<0.05); the periodontal pocket depth, bleeding index and plaque index of the observation group was significantly lower than the control group, with statistically significant difference (all P<0.05). At half year and 1 year after treatment, the patients′ satisfaction in the observation group reached 91.40%(85/93) and 94.62%(88/93), respectively, which was also significantly better than that in the control group [81.72%(76/93), 89.25%(83/93), all P<0.05]. Conclusions:Compared with the conventional periodontal splint, the combination of super fiberglass periodontal splint can significantly improve the treatment effect and patient′s satisfaction.

ABSTRACT

This article describes cases of applying non-surgical treatment including scaling and root planing, occlusal adjustment and tooth splinting of periodontally compromised lower anterior incisors Clinical and radiographic evaluations were performed over a 1–3-year period. All clinical parameters and radiographic bone levels improved in both cases. Dramatic regeneration of alveolar bone and lamina dura were observed on radiographic images, and no specific complications occurred during the follow-up period. Within the limitations of this study, these cases demonstrated the possibility of tooth rescue through non-surgical treatment and splinting of periodontally compromised teeth typically considered for extraction.

ABSTRACT

Objective To study the effect of unmovable splint therapy in patients with moderate and severe periodontitis. Methods Twenty periodontitis cases with loosened anterior teeth were treated with unmovable splints after the basic periodontal therapy. The clinical effect and periodontal index were evaluated in the sixth month, the first year, the second year and the third year. Result The success rate was 90 percent three years after the restoration.Conclusion Unmovable splint therapy is one of the effective therapies in the treatment of periodontitis.

ABSTRACT

BACKGROUND:It is believed that the splintage can promote periodontal tissue repair and improve the prognosis of loose teeth. However, the force of the splint is associated with the rigidity of the splint and adaptation between the teeth and the splint. OBJECTIVE: To evaluate the therapeutic effects of the nylon composite resin periodontal splint, the fiber-reinforced composite periodontal splint and the bracket periodontal splint on the treatment of severe periodontitis of the mandibular anterior teeth. METHODS:Seventy-two patients with severe periodontitis of the mandibular anterior teeth were randomized into 3 groups, with 24 cases in each group: nylon composite resin periodontal splint, fiber-reinforced composite periodontal splint and bracket periodontal splint groups. The clinical effects of the three kinds of periodontal splints and periodontal indexes were compared at 1, 6 and 12 months after splinting. RESULTS AND CONCLUSION: The clinical effects in the group of the fiber-reinforced composite periodontal splint were significantly better than those in the other two groups at 1, 6, 12 months, which were 100%, 100% and 95.8%, respectively. The plaque index, bleeding index, and pocket depth were worse in the group of the nylon composite resin periodontal splint than the other two groups (P < 0.05). Results suggest that the fiber-reinforced composite periodontal splint is preferred to preserve the loosened teeth with severe periodontitis.

ABSTRACT

Fundamento: el tumor más común de las glándulas salivales es el adenoma pleomórfico. Es un tumor mixto que se presenta con frecuencia en la glándula parótida. Objetivo: presentar las características clínicas e histopatológicas de un adenoma pleomórfico localizado en el paladar. Se describe también el tratamiento realizado. Caso clínico: se presenta el caso de una paciente con un adenoma pleomórfico localizado en el paladar duro. El tumor de 7 cm por 5 cm se encontraba cubierto de mucosa normal firme, en algunos sitios con poca resiliencia, gomosa, no dolorosa; además los dientes de la zona presentaron movilidad. De acuerdo con la radiografía se observó una zona esclerótica difusa y desde el punto de vista histopatológico se detectó la presencia de nidos, cordones y estructuras ductales, con una secreción luminal eosinofílica. A la paciente se le realizó una maxilectomía parcial y colocación de férula intraquirúrgica. Conclusiones: aunque el adenoma pleomórfico es un tumor benigno requiere un tratamiento quirúrgico con resección de un margen de tejido sano debido a que tiende a infiltrar la cápsula. En el presente caso su dimensión fue tal que requirió una maxilectomía parcial.


Background: benign pleomorphic adenoma is the most common tumor in the salivary glands. It is a mixed tumor that frequently appears in the parotid gland. Objective: to present the clinical and histopathologic characteristics of a benign pleomorphic adenoma located in the palate. Clinical case: the case of a patient with a pleomorphic adenoma in the hard palate is presented. The tumor, that was 7 cm by 5 cm, was covered with a normal firm mucosa which had not much resilience and was rubbery and non-painful. Besides, the teeth in the area presented mobility. According to the radiography, there was a diffuse sclerotic region and from the histopathologic point of view, nests, cords, and ductal structures were detected, along with a luminal eosinophilic secretion. The patient underwent a partial maxillectomy and the placing of an intrasurgical splint. Conclusions: in spite of the fact that the pleomorphic adenoma is a benign tumor, it requires a surgical treatment with removal of a part of healthy tissue because the adenoma tens to infiltrate the capsule. The present case, due to its dimensions, required a partial maxillectomy.

ABSTRACT

BACKGROUND:Studies have found that the glass fiber splint which is embedded into the groove on the lingual surface of the affected tooth plus fluid resin fixation between the gap of mobile teeth can achieve more satisfactory clinical effects on the stabilization of loose front teeth. OBJECTIVE:To observe the clinical effects of glass fiber splints prepared using two tooth preparation methods on splinting loose front teeth and their debonding rate through a 5-year fol ow-up visit. METHODS:Ninety-four chronic periodontal disease patients, who had received periodontal treatment, were enrol ed in this study for their loose front teeth of degree II-III. Al the enrol ed patients were good candidates for periodontal splinting strictly in accordance with the indications for periodontal splinting. According to whether the fluid resin was used to seal the gap between the mobile teeth, the 96 patients were divided into two groups:patients undergoing glass fiber splint+fluid resin between the mobile teeth served as experimental group, and those only undergoing glass fiber splint as control group. Al the patients were fol owed by regular clinical referral or telephone fol ow-up for 5 year, to evaluate the fixed effects. RESULTS AND CONCLUSION:During the 5 years of regular clinical referral or telephone fol ow-up, seven of the 76 cases and 12 of the 18 cases were found in the experimental and control groups, respectively, to have debonding between the abutment and the splint. Compared with the control group, the probing depth and attachment loss of affected teeth were improved significantly (P<0.05), and the percentage of alveolar bone height accounting for the root length increased significantly in the experimental group (P<0.05) after 5 years of fol ow-up. Patients in the experimental group felt comfortable without foreign body sensation. The results confirmed that the glass fiber splinting plus fluid resin fixation between the gap of mobile teeth was a better treatment for loose front teeth during the 5-year fol ow-up.

ABSTRACT

Objective To introduce and analyze the design, manufacture and clinical effect of the casting full dentition splint in treatment of periodontal disease with or without dentition defect. Methods 148 patients with periodontal disease during 1995 to 1998 were treated with casting full dentition splint after a thorough basic periodontal treatment. The patients were composed of 81 men and 67 women aged 40-69y. The patients were followed up for 1 to 3 years after restoration. General effects of the prosthesis were evaluated based on the chief complain of patients, clinical examination and X-ray examination. Results Among 148 patients, 140 (94.59%) were satisfied with the prosthesis and stable periodontal status, 6 (4.05%) were improved, 2 (1.35%) showed no clinical effect. Conclusion Casting full dentition splint can improve the mastication efficiency in periodontal disease patients. It is an effective and reversible therapeutic method on treatment of the periodontal disease with or without dentition defect.
